In 2014, Martin Health developed a new program to utilize pharmacists' expertise in complex medication management. The Comprehensive Health Management program expands the role of pharmacists to include direct patient care; they have become instrumental in educating and supporting patients who have multiple chronic conditions and take a variety of medications.
These pharmacists are found at the patients' bedside in the hospital and in the offices of the organization's primary care practices. Their goal is to help patients with complex chronic illnesses, such as diabetes, hypertension, obesity, and heart or lung disease, so that they are eventually able to self-manage their health.
The end result is fewer hospital readmissions and reduced emergency room visits for these patients. Ultimately, comprehensive health management improves the quality of care and overall patient satisfaction.
In 2015, Barry Sterneck found himself in the hospital and in a coma due to complications from diabetes and other chronic health problems. Today, he’s a much healthier, active 72-year-old and he credits Heather Robertson, PhD, from the Martin Health Comprehensive Health Management program, with getting him on a healthier track. “I wouldn’t be here today, if not for Heather,” he said.
Innovation of the Year in Patient Care
In October 2016, the Comprehensive Health Management program at Martin Health was recognized for its progressive approach with patients who have chronic illness and take multiple medications. The program was awarded the Florida Hospital Association's Celebration of Service Award for Innovation of the Year in Patient Care in the 150-bed category.
