Our Diabetes and Nutrition programs are one of the ways Cleveland Clinic Martin Health reaches beyond the walls of our hospitals and medical practices to improve the health of our community. By investing in activities like educational sessions, nutritional counseling and local health fairs, we are able to get to the root of life-threatening chronic diseases like diabetes, obesity and lung cancer.

The following programs complement our regular medical care by helping individuals make significant changes in their lives:

Diabetes education
Through one-on-one consultation with both a diabetes registered nurse as well as a registered dietitian, individuals are able to improve diabetes self-care skills and enhance their quality of life. Our programs for adult, pediatric and gestational diabetes are recognized by the American Diabetes Association.

Medicare Weight Management Program
The Center for Diabetes and Nutrition at Cleveland Clinic Martin Health is pleased to provide the Medicare Weight Management program to help older adults with obesity shed extra pounds and lead a healthier lifestyle.

Nutritional counseling
Individualized nutritional counseling is an outpatient service available to assist participants in learning how to eat healthy and enjoy delicious meals. In a one-hour private session, our registered dietitian will help participants plan a healthy diet based on lifestyle, medical history, food and preferences.
