CheckList- Make sure you are ready for Monday of General Orientation!
All offers of employment are contingent upon successful completion of the pre-employment process and below is an outline of the steps required to complete the process.
STEP 1 – Immediately contact our Occupational Health Department at 772-223-4960 to schedule your pre-employment visits. Occupational Health is located in the Ocean East Plaza at 2384A East Ocean Boulevard in Stuart (directions attached).
STEP 2 - Please COMPLETE AND PRINT the Orientation Self Study questionnaire using the provided Study Guide (do not print) and bring it with you to the Career Center according to STEP 4 below. This questionnaire must be completed before you can start work.
STEP 3 - 1st pre-employment visit (approx. 1 hour) Occupational Health visit. You will receive a drug test, TB test and any other pre-employment testing that is associated with your new job.
STEP 4 - 2nd pre-employment visit (approx. 30-45 minutes) will be at the Career Center in the North Hospital 200 Hospital Ave. and will occur 2 days after your 1st visit. Enter through the main lobby and let Security Officer know you are reporting to the Career Center. Be prepared to present the following documentation:
• Orientation Self Study Questionnaire (attached)
• Driver’s License and Social Security Card (no copies/must be original)
• Other employment eligibility documents as required by Form I-9.
• Professional Licenses and Certifications (no copies/must be original) as required by your job description.
• Proof of BLS (required), ACLS and/or PALS.
• Be prepared to take an initial medical surgical and basic EKG assessment
**Please note the ONLY acceptable BLS Certifications are- American Red Cross – CPR/AED for the Professional Rescuer or American Heart Association - Basic Life Support for Healthcare Provider.
STEP 5 – Begin Orientation on Monday at 8 a.m. This will be followed by initial clinical orientation starting on Tuesday thru Friday, 8 a.m. to 4:30 p.m. During this time you may wear any color scrubs. You will be given instructions at this time on what the uniform requirements will be going forward.
