Overview

Cleveland Clinic is a nonprofit, multispecialty academic medical center that is recognized in the U.S. and throughout the world for its expertise and care. The health system comprises a main campus medical center in Cleveland and 22 additional hospitals, including five in Southeast Florida. In total, there are 1,500 pharmacy employees, 20 ambulatory pharmacies, significant inpatient and outpatient clinical services and a pharmacy budget exceeding $1 billion.

In Florida, there are currently 6 PGY-1 pharmacy residency programs with one program being completed mostly in the ambulatory care environment. Our residency programs provide education and training with a primary emphasis on the development of skills in specialized pharmacy practice areas. Throughout the year, residents are involved in service, education and research activities. During this training, residents learn to conduct patient care activities using a consistent approach that reflects the philosophy of pharmacy care across the continuum. In addition to clinical activities, residents are involved in educational programs, including pharmacy conferences, journal clubs and a seminar presentation. Teaching opportunities include medical and nursing in-services, as well as co-precepting students from colleges of pharmacy. Research skills are developed through experience and enhanced by instruction as residents complete a research project and/or drug use evaluation. PGY-1 residents attend the ASHP Midyear Clinical Meeting, a residency conference, and have the opportunity to attend and participate in other state and local conferences.

PGY-1 programs

Interested applicants must be PharmD graduates of accredited college of pharmacy, be eligible for licensure in the state of Ohio (or Florida if applicable), and submit the standard application materials through PhORCAS (i.e. letter of Intent, CV, the three standard reference forms in PhORCAS, official college transcripts), along with:

We request a minimum of one (two is preferred) of your three references should come from a preceptor who you have worked with in a clinical setting, related to an APPE in acute or ambulatory care. The clinical preceptor should be able to comment on your scope of responsibility, total patient load, level of autonomy, clinical abilities, and organizational and time management skills. All three reference writers should use the standard PhORCAS template to submit their candidate recommendation.

A response to the following essay statement is required for acceptance. The essay response should be no longer than one page in length. An application will not be reviewed if this statement is missing. Please note that the essay is separate from your letter of intent. Reflecting on your APPE rotations, please provide a piece of critical feedback you have received from one of your preceptors on a direct patient care rotation. Explain how you have incorporated this feedback into your practice and/or worked to improve in this area.

Please note: Residency applicants must be authorized for employment in the United States at the time they apply for the residency. Cleveland Clinic Pharmacy does not sponsor applicants for work visas.

Appointments of applicants to residency positions may be contingent upon the applicants satisfying certain eligibility requirements [e.g., graduating from accredited college of pharmacy, obtaining pharmacist license in Ohio (for Ohio sites) or Florida (for Florida sites) within 90 days of start date, and successful completion of a pre-employment physical and drug screen, including testing for cotinine and attendance of an onboarding appointment on-site at Main Campus or as otherwise directed by Occupational Health].

Unfortunately, we cannot accommodate OPT (Optional Practical Training) Visas for the Cleveland Clinic Pharmacy Residency Program as the program extends beyond 12 months. TN (Trade NAFTA) Visas are acceptable, however the candidate is responsible for the application process and all necessary fees. Cleveland Clinic cannot extend the candidate an employment letter until the candidate has matched with the Cleveland Clinic program and signed an acceptance letter. Any employment letter will state the length of the program (12 months if PGY1 or PGY2, 24 months if HSPAL or Pharmacotherapy).

Graduates of international pharmacy schools will be considered if they meet the ASHP criteria for application to residency (“Applicants to pharmacy residencies must be graduates or candidates for graduation of an Accreditation Council for Pharmacy Education (ACPE) accredited degree program (or one in process of pursuing accreditation) or have a Foreign Pharmacy Graduate Equivalency Committee (FPGEC) certificate from the National Association of Boards of Pharmacy (NABP). At a minimum, the program must be a five-year pharmacy degree program”).

Cleveland Clinic requires all employees to demonstrate proof of full COVID-19 vaccination subject to approved medical or religious exemptions.

The Pharmacy Practice Residency (PGY1) is an American Society of Health-System Pharmacists (ASHP) accredited, one year, post-graduate program offered at Cleveland Clinic Florida’s state-of-the-art medical campus in South Florida. The pharmacy residency was established in 2007 with the goal of achieving the following for residents and the community:

  • Develop confident and proficient pharmacists who are able to provide direct pharmaceutical care.
  • Establish a strong service-oriented work ethic that embodies professionalism, excellence and integrity.
  • Advance the practice of pharmacy in our institution and community.

Program description

The Cleveland Clinic Weston PGY1 residency program provides education and training with a primary emphasis on the development of practice skills in a number of adult pharmacy practice areas. Throughout the year, the residents are involved in a variety of service, education, and research activities. During this training, the resident will learn to conduct patient care activities using a consistent approach that reflects the philosophy of pharmaceutical care. Cleveland Clinic Weston is comprised of the hospital facility located in Weston Florida and clinics located in West Palm Beach, Coral Springs, Palm Beach Gardens, Parkland and Wellington. It is a not-for-profit, multi-specialty, academic medical center that integrates clinical and hospital care with research and education. Cleveland Clinic Weston has more than 250 physicians with expertise in over 50 specialties. The medical campus is fully integrated and includes diagnostic centers, outpatient surgery and a 24-hour emergency department located in the state-of-the-art hospital. Cleveland Clinic Weston is a 256-bed community teaching hospital. Cleveland Clinic Weston is a not for profit multi-specialty group practice with approximately 35 specialties dedicated to providing outstanding, state of the art medical care. Pharmacy Practice Residency (PGY-1) features

The one-year Pharmacy Practice Residency Program (PGY-1) provides a foundation of diverse experiences in pharmaceutical care as well as in-hospital pharmacy practice. The pharmacy residency program is designed to expose the resident to a variety of practice areas in order to build and expand the knowledge and skills necessary for direct patient care. Resident will confidently and professionally integrate themselves into the health care team and utilize evidence-based medicine to optimize drug therapy outcomes. They will conduct practice related research and have multiple opportunities to develop skills in educating healthcare professionals and the community. Residents who successfully complete this program are well prepared for specialized residencies, fellowships or other career opportunities.

The pharmacy practice residency is designed to provide in-depth training in multiple aspects of hospital pharmacy including: pharmacokinetics, nutritional support, oncology, critical care, pharmacy administration, infectious disease, and ambulatory care. Residents’ expectations include, but are not limited to, preparing in-services, MUEs, and completion of a longitudinal residency project. The PGY-1 program is designed to meet each resident’s interests and needs and assist them in developing as clinical pharmacists. Required rotations in acute care include internal medicine (2 months), critical care (2 months), emergency medicine (1 month), and additional rotations in administration (1 month), clinical research (1 month), and orientation (1 month). Residents will have the opportunity to select 4 months of electives from the following: pediatrics, nephrology, diabetes management, emergency medicine II, oncology, infectious diseases, medication safety, academia, or repeating a core rotation.

Cleveland Clinic Martin Health precepts pharmacy students from a number of pharmacy schools in the area, including University of Florida, Nova Southeastern University, Palm Beach Atlantic University, and the University of South Florida. Residents will have the opportunity to precept and develop teaching skills through interaction with the students. A formal teaching certificate program is also available for residents with advanced interest in academia.

The PGY-1 pharmacy residency programs at Cleveland Clinic Martin Health (CCMH) are conducted at Cleveland Clinic Martin North, Cleveland Clinic Martin South, and Cleveland Clinic Tradition Hospital. One of the benefits of the PGY-1 programs within CCMH is the ability of the resident to complete learning experiences at all hospitals within the system.

PGY-1 residency schedule

Residents will complete twelve one-month rotations during the course of the year.

My immediate goal is to build upon my experience as a clinical pharmacist. I also plan on specializing in infectious disease by taking the BCIDP while continuing my learning, and developing my clinical skills closely with an antimicrobial stewardship program. I hope to further develop my preceptorship skills working with advanced pharmacy students’ in acquiring the necessary clinical skills for future practice.

Why Martin: Cleveland Clinic Martin Health has a wealth of dedicated preceptors with unique specializations that span across nephrology, oncology, infectious diseases and with other possible rotations available in the other Cleveland Clinic locations across the Florida region. The institution prides itself in collaborative learning with other healthcare professionals which is something I found particularly alluring. This program has availed me opportunities to round with, discuss clinical initiatives, present and collaborate in research with physicians and other specialists. It has also provided me with the right work and life balance, while challenging my abilities as both a pharmacist and a resident.

I enjoy spending time outdoors and walking around the beach.

Why Martin: After having the privilege of enhancing my learning at Cleveland Clinic Martin Health during my APPE rotations for several months next to the amazing preceptors, I solidified my interest in this residency program. Our health system is composed of 3 hospitals, a stand-alone emergency department, as well as the Robert & Carol Weissman Cancer Center, rehabilitation services, and numerous primary care clinics. I also hope to get a certificate in teaching to be able to have an adjunct faculty position at a college of pharmacy. Ultimately, I want to advance my clinical knowledge and gain more experience as a pharmacist to provide excellent patient care.

Why Martin: I had the opportunity to do both IPPE and APPE rotations at Cleveland Clinic Martin Health and was captured by how passionate my preceptors were about my learning experience. The pharmacist I encountered at CCMH showed how dedicated they were to patients and allowed me to gain firsthand experience in patient care. My positive experience at CCMH was the primary driving force behind pursuing a pharmacy residency with the ultimate goal of becoming a competent and autonomous clinical pharmacist.

The PGY-1 residents will practice at Cleveland Clinic Tradition Hospital. The Department of Pharmacy provides comprehensive pharmacotherapeutic services throughout the health system using a decentralized staffing model. Required rotations include internal medicine, critical care, emergency medicine, antimicrobial stewardship, administration, medication safety, and research. Residents will also choose four electives from the following list: pediatrics, nephrology, oncology (inpatient and outpatient), transitions of care, infectious diseases, medication safety, academia, and repeating a core rotation.

Residents will complete a research project, manuscript, and present at the following venues: ASHP Midyear Clinical Meeting and Florida Residency Conference. Residents are required to staff one evening weekly, one weekend every three weeks, and work two holidays. Other longitudinal learning experiences include ACLS and BLS certification, medication use evaluations, student precepting, and participation in pharmacy committees. To assist with building research skills, the residents will participate in the residency research curriculum that takes place throughout the residency year. Formal teaching and leadership certificates are available for interested residents.

Practice site overview

Cleveland Clinic Indian River Hospital was founded by Garnett Radin, R.N., in 1932. Cleveland Clinic Indian River Hospital is a 332-bed, not-for-profit hospital offering comprehensive medical services to the Treasure Coast community. The Welsh Heart Center, Scully-Welsh Cancer Center and Scully Endoscopy Center offer full-service programs and a multidisciplinary approach. Healthcare professionals represent specialties including orthopedic services, minimally invasive vascular surgery, neurosurgery, stroke services, robotic surgery, a state-of-the-art wound healing center, and the only maternity wing in the community. The hospital features all private rooms and is the largest private-sector employer in Indian River County. The Department of Pharmacy provides 24-hour service with a professional staff of approximately 32 pharmacists with 16 being board certified in their area of practice. Pharmacists are actively involved within ambulatory care, emergency services, the intensive care unit, antimicrobial stewardship, outpatient cancer center, behavioral health center, and decentralized on the general medical floors. Pharmacists also serve as pharmacy student preceptors and provide patient counseling.  

Programs overview

There are two PGY-1 Pharmacy Programs at Indian River Hospital. One program (program code 33850) is an accredited program with two positions. Most required and elective learning experiences occur in the inpatient setting. The other program (program code 33938) is a candidate status program with one position that meets all requirements of a PGY-1 pharmacy program, but most required and elective learning experiences occur in the ambulatory setting. PGY-1 pharmacy residency, inpatient setting

(program code: 33850)

Residents will practice in various inpatient pharmacist areas at Cleveland Clinic Indian River Hospital, including intensive care units, the emergency department, cardiology and internal medicine floors, the main pharmacy area for operational services and sterile preparation, ambulatory care clinics specializing in anticoagulation and chronic care management, as well as optional forays into the specialized behavioral health center for psychiatric pharmacy or the Scully-Welsh Cancer Center for a hematology/oncology experience. 

Residents will complete a research project, manuscript, and present at the following venues: ASHP Midyear Clinical Meeting and The Florida Residency Conference. Residents are required to staff every third weekend.  Residents are also required to work two holidays. Other longitudinal learning experiences include research, leadership, and operational pharmacy practice. To assist with building research skills, the residents will participate in the residency research curriculum that takes place throughout the residency year. Optional Experience: teaching certificate program.
